Dear Parents/Guardians:

These pages will provide some important information regarding your student's health and well-being while at school. I will do my best to provide current and up-to-date announcements regarding health information, as well as any forms that you might need throughout the year.

Please note that Pennsylvania State Law requires all incoming kindergarten students require a physical examination and all 3rd graders a dental exam. As always, these can be provided - free of charge - through the school physician and dentist. All students will be examined for height, weight, and vision. If a student requires medicine during school hours, please fill out the form and turn the medication into the office with the Medication Authorization form, which must be signed by the prescribing physician. The medication must be in a bottle labeled from the pharmacy. You can drop off the form or fax it to (814) 474-2719. Any over-the-counter (OTC) medication must be sealed in a container that is clearly labeled and any medication that is taken longer than ten days will require a physician's authorization.

Your student's health is very important to me. I encourage you to call my direct line, (814) 474-3109, with any comments, questions, or concerns. I am usually in my office and able to take calls; however, I do have to step out occasionally. If I am out, please don't hesitate to leave me a voicemail. My voicemail is private, so confidential information will not be shared with anyone other than myself. I check my voicemail quite frequently and am often able to get back in touch with you within a few minutes. If I am out and you find yourself in an emergency situation, please contact the front office at (814) 474-3123.
